Community Wellness Partners is a partnership between Cottonwood, Jackson and Nobles counties to promote wellness throughout the three-county region. Our goal is to create healthy environments in our region to make the healthy choice the EASY choice!
Healthy Eating and Physical Activity in Schools
We work alongside school staff to create permanent changes in lunchrooms and offer opportunities for physical activity in area schools.
Health Eating and Physical Activity in Communities
From creating community Active Living Plans to assisting area restaurants to create healthier menu options, CWP offers assistance and funding to encourage communities to eat healthy and stay active.
Wellness Works
We partner with local employers to promote creation of healthier worksites for employees through changes in policies, systems, and environments. Areas of focus include nutrition, physical activity, tobacco cessation/exposure and breast feeding support.
Tobacco Cessation
Smoke-free housing and limiting second-hand smoke exposure is a right for all ci
tizens. CWP works with housing managers and residents to ensure smoke-free environments for all housing communities.
Diabetes and Hypertension Prevention
Community Wellness Partners coordinates efforts with healthcare systems and local organizations to raise awareness of pre-diabetes and hypertension. With an estimated 89 million people at risk for pre-diabetes and one of every three American adults at risk for prehypertension, these are both increasing concerns for our residents.
Community – Clinical Linkages
Ensuring that everyone has the ability to be healthy is a top priority for Community Wellness Partners. That’s why we’ve hired two Community Health Workers, who work with individuals and families to help them navigate health care and social services systems. We also partner closely with health systems to support preventative care efforts and referral to community resources.
