Your opinion about the health in our entire community is important, so we are asking for your help.

Sanford Worthington Medical Center is collaborating with Nobles County Community Services-Public Health to conduct  a community health needs assessment survey. This process assists the community leaders in understanding the health status of the area and identifying priority health issues affecting Nobles County residents.

By participating in this community health needs assessment survey, you can help shape the future health, well-being, and quality of life of residents in the area. The survey is about general health and wellness concerns of Nobles County residents. The information gathered from this survey will help us identify unmet needs in the community. The survey results will also help to develop plans to address any gaps in services.

You must be 18 years of age or older to participate in the survey. Please take a few minutes of  your precious time to complete the survey. It should take you no more than 20 minutes to complete.
