Medical Assistance recipients requiring Personal Care Attendant services need an assessment by the county Public Health Nurse (PHN) in order to access the program. Annual reassessments by the county PHN are required to continue those services. This also includes those recipients receiving PCA services on the MR/RC Waiver. The County PHN is authorized to decide the appropriate level of care, develop a service plan, and recommend the necessary amount of PCA service and RN supervision of PCA services. This is a mandated program in Minnesota.
Seniors may have health care needs that require more than visits to a clinic or a short stay in a hospital. Medical care may be needed over a long period and may be delivered through home-based care, assisted living or a nursing home.
Nobles County Public Community Services Health Nurses play a key role in assessing what kind of long-term health care is best for a senior’s situation. For example, a preadmission screening by a Public Health Nurse is a requirement before admission to a nursing home.
The Long-Term Care Consultation is an evaluation done at the senior’s home by a Public Health Nurse. The purpose of the consultation is to assess the health care and medical needs of the senior. The result of the consultation is a comprehensive set of recommendations the senior and family members can consider for maintaining the health and care of an individual. The screening is a requirement for all admissions to a nursing home.
